Establishing a line of credit with ADL Supply is easy. Just click on the link below to download and print the credit application. Please fill out and email or fax back the completed application to (305) 402 - 0280. Your application will be processed within 5 business days.
Application must be signed and dated in order to be processed. Additional information may be required in order to open a line of credit.
We only issue open lines of credit to businesses. Please make sure to include a list of references with your application.
By filling out the credit application you represent yourself as a municipality, corporation, or government agency. ADL Supply reserves the right to terminate your open line of credit immediately if we determine you are not a business and have misrepresented yourself as a business.
